BHS BAND FIELD TRIPS PERMISSION FORM
I do hereby give permission for my son/daughter, or ward to attend field trips,
events, and travel as a member of the Brighton High School Band. I understand
that I must provide funds for my child’s meals during said trips. I understand
any misconduct will result in suspension of my child/ward from school. I hereby
waive all rights to hold the Tipton County Board of Education, Brighton High
School or any part of its employees responsible in the event of an accident or
injury occurring on these trips to the student whose signature appears below.
